FOR FAMILIES SEEKING NURSING HOME CARE:
We can help you choose “just the right” nursing home for your loved one. We have up to date information on eligibility requirements and the referral process. We can answer your questions about Medicare, Medicaid and Long Term Care Insurance.
Despite the limitations that are currently imposed in care settings, our team is working collaboratively to implement creative adaptions that will help to ensure that our Social Workers and Registered Nurses can continue to meet the needs of our clients. These include telephone calls, use of a simple, personalized tablet and video conferencing in place of in-person visits. As New York State is now in Phase 4 of the re-opening guidelines, we offer an option of socially distanced “in person” visits.
